The following guide was developed by members of the Transition Continuum Workgroup, a sub-committee of the Pima County Children’s Council. It is our hope that this guide will assist professionals working with adolescents and young adults to recognize the unique needs of this population and to ensure that youth have adequate preparation to transition into adulthood.
